Model Consent Form

Hereby:

I assign to the Producer all and any intellectual property rights in full to the intellectual property objects created by me during the Video filming: the performance of my role as part of the Video, the recording of the performance, my image, voice, name, fragments and elements of the performance / recording of the performance, both included and not included in the final version of the Video (hereinafter referred to as Performance).

I give my consent to the independent and fragmentary usage of the Performance, its modifying, making the Performance available to the public for the first time, inclusion in whole or in fragments into the complex or composite works, publication, creation of new or derivative works based on the Performance, use of the Performance without specifying my name (anonymously).

I confirm that I have no right to use the Performance by my own, as well as the script of the Video and the Video, as well as to grant any rights to use them to third parties.

I irrevocably give my consent to the Producer to use my photo and video images included in the Performance, sound recordings at his discretion, without limitation of time, without limitation of the territory of use, including for commercial purposes, in any way not prohibited by law, by independent or fragmentary usage, modifying, making the images available to the public for the first time, inclusion in whole or in fragments into the complex or composite works, publication, creation of new and derivative works based on them, use without specifying my name (anonymously), without any reports on their usage, with the right to transfer to third parties.

I am aware that a Video with my participation can be used for the Client's advertising purposes.
